Caregivers are frequently torn between family, work and personal obligations, and the aging loved one experiencing diminished energies and physical challenges which may place them at-risk. Changes are often subtle and gradual, however, with some individuals change may be unpredictable and severe.
Independence and safety can be enhanced and prolonged with proper support, monitoring and good caring professionals.
Case Managers are responsive to distance caregivers who cannot rearrange their lives and need the security of knowing that those they love are safe and secure. Case Managers can assist with medication management and compliance. They can provide assistance and guidance with financial matters, and arrange for transportation and escort to health care appointments. They to serve as reliable and dependable liaisons for caregivers.
SERVICES - Case Manager Professionals
Perform individual needs assessment and complete intake applications
Develop a Plan of Care with the individual and their caregiver. This plan is based upon assessment and desires of the individual in agreement with the caregiver as appropriate.
A written Task Agreement will outline specific tasks and schedule of services that will be performed.
Case Managers will utilize their knowledge of community resources and make appropriate referral(s) to accommodate personal choice and provide the greatest array of support.
All services will be monitored by a professional to assure the highest quality of care and delivery.
Updates and reports will be provided on a regular basis or as needed by distance caregivers.
Case Managers will provide Telephone Counseling as needed.
MOST FREQUENTLY REQUESTED SERVICES
Personal Care-Certified Nursing Assistants (CNA's) will provide professional and caring assistance with bathing, shampooing, grooming and dressing.
Homemaking-Light meal preparation, assistance with laundry, and housekeeping to keep home tidy, sanitary and free of fall hazards.
Respite and Companionship-Provides an opportunity for caregivers to run errands, take a walk, read a book or just rest without interruption or worry.
Shopping Assistance - Help to maintain and replenish pantry and household items. This may also include picking up prescriptions and Health Care supplies.
Escort - Accompanying an individual to health care appointments. Workers may act as a liaison by sharing with Caregivers, instructions and information which may not be easily understood by the individual.
Emergency Response Systems - Installed and monitored by professional staff for individuals who do not feel secure living alone or for frail persons with health related problems or at-risk of falling.
Distance Caring - The unique needs of Long Distance Caregivers are assigned to highly trained and experienced professionals. All staff are trained to observe and immediately report any changes in the individual's situation or health to the Case Manager. The Case Manager is responsible for monitoring and responding appropriately. Distance Caregivers have a professional, dedicated and caring representative available to respond on their behalf in time of need.
