Crestview Manor is a Private, Non-Profit, 501(c)(3) Facility. The building is committed by the Okaloosa Board of County Commissioners to provide affordable supportive living for low and modest income elderly, disabled, or recuperating persons. We were established to provide a safe living alternative for persons who are not secure living alone. Residents are encouraged to function at their highest level of personal ability. Level of assistance we provide is in accordance with the State guidelines and the personal needs of each resident.

Persons exceeding standard care guidelines may be eligible to remain at Crestview Manor by enrolling in Extended Congregate Care. Resident and Sponsor will be involved in all decisions.

  • Utilities

    All utilities are furnished except for cable TV, internet, and telephone. These services are available in each room by private arrangement. A common room TV is available for residents to watch if resident chooses not to get cable in their room.

  • Resident Rooms

    Rooms may vary in size and shape. Residents are encouraged to use their own furnishings to personalize their area making it feel comfortable and more like home. There is no additional charge should facility furnishings be used by residents.

  • Appliances

    No small appliances are allowed in resident rooms, however, a community kitchen is provided and may be utilized by residents. Meals are in a family style dining area and coffee, water, and tea are provided throughout the day so appliances are not necessary. ‚Äč

  • Fees

    Fees include the services listed below and begin at $2200 for double occupancy, $2600 for a private room with a shared bathroom.

The facility is located approximately three miles from a hospital and several physician office complexes. Staff will assist residents in arranging appointments as the need arises.
Transportation and an escort is available, should the resident desire it, for an additional cost
In the event of an emergency, the County's 911 emergency services will be called.
Level of Care Category is regulated by State guidelines and in consultation with attending Physician, Facility Director, Resident Sponsor, and CARES.


Personal Care
Personal Care

Help with bathing, shampooing, and dressing is provided as a service to residents. The degree of assistance provided is based on individual need and level of care as outlined in State guidelines for Activities of Daily Living.

Meals

Menus are developed with a registered dietitian to assure nutritional balance. Salt, sugar, and fats are restricted in meal preparation. Individuals are free to season foods to taste at the table.
Meals are served in the main Dining Room and residents are expected to go. Temporary room service is provided for persons returning from a hospital stay or experiencing a short-term illness.

Each resident should maintain order in their personal area. The level of assistance will be determined by an individual assessment of the residents capabilities by the Facility Director.

Activities

Social interaction among residents is highly encouraged to help residents remain cognitive and to help make retirement more enjoyable. Planned activities include BINGO, crafts, board games, and community outings/field trips and more. Many of these activities are sponsored by local churches, organizations, groups, or individual volunteers to enhance the quality of life for residents. Special events are planned throughout the year by a community caring for it's elders.

Medication Supervision

Our Facility Staff Members are not licensed to administer medications; however, they are trained to supervise and record self-administration of medications by the residents. Medications will be kept and received in a centrally located area in individual containers. Residents are required to come to the site at designated times to receive their doses as prescribed by their Health Care Providers.

NO PATIENT LEFT ALONE

In accordance with SB 988, the facilities guarantee Florida families the fundamental right to visit their loved ones who are receiving care as well as the right to be hugged by their loved ones.

The facilities will allow for in-person visitation in all the following circumstances, unless the resident objects:

  • End-of-life situations.
  • A resident who was living with family before being admitted to the facility's care is struggling with the change in environment and lack of in-person family support.
  • A resident is making one or more major medical decisions.
  • A resident is experiencing emotional distress or grieving the loss of a friend or family member who recently died.
  • A resident needs cueing or encouragement to eat or drink which was previously provided by a family member or caregiver.
  • A resident who used to talk and interact with others is seldom speaking.

Additionally, the facility allows a resident the option to designate a visitor who is a family member, friend, guardian, or other individual as an essential caregiver. In-person visitation shall be allowed by the essential caregiver for at least two hours daily in addition to any other visitation established by the Facilities.

Visitors may visit their loved ones any day of the week. We do ask that visitors consider the resident's need to rest and plan visits between the hours of 8:00 AM and 4:00 PM. For the safety of residents and staff, doors are secured in the early evening hours. Entry may be granted by noticing staff in the later hours.

The facility will post publicly procedures for infection control to include handwashing and the use of approved hand sanitizer. An infection control "station" will be established at the front entrance. Staff will monitor visitors for compliance and provide direction and guidance. The most current written information will be available to all visitors.

All visitors will complete a health screening upon arrival. Staff will advise visitors of the need for protective equipment as well as compliance with the most current health advisories.

The number of visitors will be dependent on several factors: Number of residents in house, number of infected residents, location of visit (resident room vs. common area), and regulatory recommendations. Staff will remain updated on the most accurate information as it pertains to the specific crisis.

The Facility Director will be responsible for staff adherence to visitation policies and procedures.

Consensual physical contact shall be allowed between a resident and a visitor.
